NHS App API - support headings in in-app messages

The subset of markdown supported by NHS App in-app messages does not currently include headings.
For lengthier messages, this makes it impossible for recipients using VoiceOver to understand the structure of the page and skip through the content, identifying relevant information.

Introduce support for headings within the in-app messages sent through the NHS App API to improve accessibility for users using VoiceOver.
