This will enable patients, especially those with complex medical histories, to provide healthcare professionals the information they need especially if they end up in A&E.

As a patient I have three different applications plus sometimes by post depending on provider/clinic. It would be very useful for all to be visible in a single app and the NHS app is the obvious candidate for patients and all those delivering care.
